EducationPostgraduate Medical Education In United Kingdom 1 PLAB REGISTRATION OF OVERSEAS QUALIFIED DOCTORS Background The Medical Act of 1978 empowered the GMC to require all overseas doctors applying for full or limited registration to show their proficiency in the English language. It also, from February 1979, replaced the system of temporary registration by a better and more flexible system of limited registration. Now, what were the TRA Baord and TRAB exam have become the professional and Linguistic Assessments Board (PLAB) and PLAB exam. Limited Registration Application forms and leaflets explaining the requirements for limited registration and for full registration are available from the GMC for this information you should write to General Medical Council Overseas Registration Division 25 Grosfield Street London WIP 8BP These regulations are fairly complicated and you must read them for youself although the summarised main points regarding limited registration are given below : 1. Limited registration can be granted for a maximum of 5 years. 2. It is only granted for employment under the supoervision of another doctor who is fully registered. 3. It may be granted for only one parpticular employment or for a specified range of employments. 4. It is only granted if, since qualifying, you have completed, outside Britian,, an internship of ast least 12 months' rudation that is recognised as satisfactory by the GMC 5. It is only granted if the GMC is satisfied that you. (a) have been selected for employment in an approved hospital or other institution. (b) have a medical qualification which the GMC accepts as satisfactory for limited registration. (c) are of good character. (d) have the necessary knowledge of English and the professional knowledge, skill and experience which are necessary for practice here. No doctor will be accepted as fulfilling requirement 5(d) unless he has passed or been exempted from the PLAB exam which is designed to test both English proficiency and professional knolwedge and competence. Exemption from the exam may be granted in individual cases: A. On the grounds of holding. (i) suitable primary qualifications (e.g. granted in UK or USA) (ii) additional registrable qualifications (e.g. MRCP, FRCS) B. On ground sof sponsorship or training by (i) British Council (ii) Association of commonwhealth Universities (iii) World Health Organisation (iv) Council of Europe (v) Technical Co-operation Training Agency (vi) International Atomic Energy Agency C. On grounds of prearranged employment in th egrade of registrar or above in a British medical school or approved hospital. D. By committee decision in certain individual cases, e.g. doctors who have completed full time courses in Britain as postgraduate students. However for exemption othe doctor must still fulfill all the other criteria for limited registration and the GMC must have received a certificate from th sponsoring body or the consultant who will supervise the practice in prearranged employment assuring them that the doctor has the necessary knwoledge of English. It is important to note that any exemption given is only in relatin to the appointment or appointments that have been arranged for the doctor or recognised by the sponsoring body or in relation to the clinical post to which he was appointed before his arrival in Great Britain. If the doctor wishes to undertake any other employment which is neither sponsored nor prearranged then he will normally then be required to pass the PLAB exam. Everyone else must sit the exam and pass it before they can be given limited registration and be allowed to work in Britain. Eligibility to sit the exam. To be eligible to sit the exam you have to be a qualified doctor and have completed a 12 months' overseas internship which is accepted by the GMC as satisfactory for the purppose. Two years' experience in any hospital in any branch of medicine or surgery except pathology, anaesthetics or psychiatry, will normally be accepted as the qequivalent of the approved internchip. The GMC is the definitive authority on eligibility and you must consult with them. Application forms Write to the GMC about registration. They will send you information which includes Note LRI Test for limited registration. Your application should be sent to the Registrar of the GMC and must unclude: 1. A completed application form (LRI Test) 2. The original diploma issued by the univeristy or other body which granted your registrable medical qualifications or an attested, signed, stamped authenticated, original certifiacte from your university stating you registrable qualifications (or eligibility to receive them) 3. If you qualified in India or Pakistan - an original certificate showing the name of the medical college at which you studied and the dates you started and finished. 4. A remittance of the test fee (at present pounds100) All documents must be in English or with attached certified official English translation. They must be sent welll in advance of the date of the exam you wish to take (the closing date of admission to each exam is ten days before it), as the waiting may be long and up to 6 months delay can occur. Dates Venues There are generally 11 exams per yera at approximately monthly intervals. The exams take place in London, Edinbrugh and Glasgow. Areas of knowledge tested. Your English proficiency is assessed as is your professional knowledge and competence. medicine, surgery and obsterics and gynaecology are the main subjects overed by the exam but other branch of medicine may be touched on and, especially in the multiple choice question paper, your knowledge or basic sciences will be tested. Exam format There are five parts in the exam : A. a miltiple choice question paper 90 minutes. B. a comprehension of spoken English paperp 30 minutes C. a medical short answer paper 90 minutes D. a written English paper 75 minutes E. as oral examination 20 minutes Parts A anc B take place in the morning and parts C and D in the afternoon of the first day of the exam (a Monday). The oral exam will follow on the Tuesday or Wednesday and you will be given your oral day and time at the test centre. Although the exam is in five parts it is also, in fact, marked and assessed as two separate exams. Your therefore sit an English language component and a medical component. The division is : A. Language component (i) Comprehension of spoken English test. (ii) written English paper (iii) part of an oral examination B. multiple choice question paper. (iv) medical short answer paper. (v) part of an oral examination. All these parts of the exam are fully explored in their individual chapters. marking Section (i) and (iv) are marked by computer. Section (ii) is marked by language examiners. The remainder of the exam is assessed by medical examiners. although language examiners may participate in the oral examinations. The marking is done on a system of grades so that : Grade 5 = excellent pass Grade 4 = good pass Grade 3 = adequate pass Grade 2 = marginal fail Grade 1 = severe fail Results As soon as possible after the exam the GMC will notify you, by letter, whether you have passed or failed. A pass means that on obtaining a suitable job in the United Kingdom you may apply to the GMC for limited registration, Failure means you cannot become registered but you can applyto resit the exam at a future date. your will be told if you have failed the language or medical components or both and if you have failed marginallly or severely. A severe failure means that you cannot retake the exam for at least six months while a marginal failure means that you cannot retake the exam for at least two months. If you pass one component but fail the other then you only have to retake the component you failed. Examination Pattern A) Medical Section The Multiple Choice Question Paper or MCQ The MCQ paper is designed to test factual medical knowledgeand recall. It is not specially designed to test english language knowledge and the wording of the question is kept as straightforward as possible. Obviously, unless your English knwoledge is good engough to understand the questions you cannot show the standard of your medicine. There are 60 questions, each of five parts, to be done in the one and a half hours allotted. Each question consists of a stem statement followed by five items or completions. For example : BCG Vaccination a) Should be given intradermally b) If positive produces a skin flare c) Uses a live vaccine d) is not effective in infancy e) Has no local adverse effects a) c) is the right answer and scores 2 a) would score 1 a) b) c) would score 1 a) b) c) d) would score 0 a) b) c) d) e) would score 1 b) d) e) would socre 3 Probable number of subject questions Medicine (M) 25 Surgery (S) 15 Obsterics and Gynaecology (O) 11 Basic Sciences (BS) 9 B) English Section This is a tapy-recorded test of comprehension of spoken English. It lasts for 30 minutes and consists of 80 questions divided into four section (each of 20 questions). You have to listen to the statement made by the voice on the tape recorded and selecte the most appropriate of four answers which are printed in the answer book you are provided with. The spoken words can only be heard and are not written in the answer book. So you 1. Listen to the tape recorded statement 2. read the four answers in your answer book 3. select the answer you think is most appropriate 4. record your answer directly onto an examination answer card which will be marked by computer. The Medical Short Answer paper This paper forms an important part of the medical examination, attracting the same number of marks as does the multiple choice paper. It takes the form of any essay question, with which most doctors are familiar from their student days. This familiarity perhaps leads to too little attention being paid to this part of the examination, though most candidates will get higher marks here than in the MCQ. You will find that you are asked to answer five out of eight set questions in one and a half (1.5) hours. Thepaper is designed to assess your ability to apply your professional knowledge to a variety of clinical situations. You may, for example be asked questions which will require you to intepret the significance of physical signs and to decide what further investigations should be undertaken, or which will demand an account of you knowledge and application of therapeutics. Questions cover mainly the three major branches of medicine - medicine, surgery and obstetrics and gynaecology. The number of questions on each subject as a percentage of the whole is, approximately, medicine 50% surgery 30% obstetrics and gynaecology 20% in the future, more enphasis will be placed upon patient management problems and therepeutics. The Written English Paper p The Paper lasts 1 1/4 hours and is marked by linguists. It tries to assess how will you can understand written English and whether or not you can write clearly and distinctly in English. It also tries to find out how good you are at expressing medicl meanings in language suitable for talking, or writting to othe doctors or for communicating with paramedical (nurses, dietician,s physiotherapists, radiographers etc) and nonmedical *patients, relatives, journalists etc) people Section A In this section you are asked to choose one from threetopics and to write 300-500 words (one to two sides of foolscap paper) about it. For example (a) Discuss the proposition that a doctor should set a good example to his patients and never smoke cigarettes. or (b) Write an article for a woman's magazine about the value of regular self examination of the breasts. or (c) Describe the advantages and disadvantages for a doctor of having clear and legible handwriting. Section B In this Section you have no choiice but will be asked to either. (1) read a passage off English and write answers to questions about its meaning. or (2) write an explanation to a nonmedical person e.g. A 50 year old man has recovered from an uncomplicated first heart attack. Write down the actual words you would use in advising him before he left hospital. or Your good friends son is thinking of becoming a doctor. Write him a letter telling him why you think this would be a good idea. In type 2 and 3 questions the number of words wanted in the answer may be stated e.g. Write 200-300 words about. Section C In this section you are given a passage of English with nearly every sixth word missed out. You have to read the passege and put signle words into the gaps so that the whole passae markes sense. The first few sentences have no gaps and, when read, provide you with an understanding of the subject being dealt with. The missing words, which you supply, have to be grammatically correct and give a logical and sensible meaning to each sentence. Oral Examination E. Your oral takes place on the first place on the first or second day after the written papers, during which you will be given your exact oral dates and time. It lastes 20 minutes and is conducted by two medical examiners who are either a surgeon and a physician, a physician and a gynaecologist, or a surgeon and a gynaecologist. They have no idea whatsoever of tyour marks in the written papers and are assessing you, independently, on your profiency in spoken English and your ability to make sensible clinical judgements.
